Reporting Injury/Workers Comp Process

If an employee gets hurt, this process needs to be followed:

  1. Call 911 if an emergency
  2. If not emergencey, employee visits School Nurse/Camp Nurse (if during office hours)
  3. Employee/Supervisor needs to inform Safety & Security so they can fill out an Incident Report eform
  4. Nurse follows up with Safety & Security with additional details
  5. If the on-site nurse/supervisor recommends employee needs a higher level of care or anything beyond minor, Nurse/Safety & Security/HR completes Authorization for Examination or Treatment form
  6. Employee takes Treatment form to one of the places listed on the Panel of Physicians for Workers' Compensation
  7. Camp/HR sends Worker's Comp form to Brotherhood via email and copies CherylF on it
    1. Email: claims@brotherhoodmutual.com
    2. Subject line: WC Incident - <Employee's name>
    3. CC: cherylf@perimeter.org
    4. Attach form
  8. Brotherhood will follow up to request details
  9. Brotherhood will send email confirmation of incident (case # and contact)
  10. Employee needs to keep HR informed of any additional treatment
  11. Any follow-up documentation needs to be sent to Brotherhood and copy Cheryl on it
